Personal Care Services
A little extra help can make a meaningful difference. Our personal care services support daily routines while encouraging each person to remain as active and independent as possible.
Depending on the individual’s needs and approved care plan, assistance may include:
- Bathing, dressing, and grooming
- Getting in and out of a bed or chair
- Walking and moving safely around the home
- Meal preparation and assistance with eating
- Toileting and other personal needs
- Light household tasks
- Grocery shopping and essential errands
- Accompaniment to appointments
- Companionship and support with daily routines
ARE YOU ELIGIBLE?
Community Choices Waiver
Louisiana’s Community Choices Waiver, often called CCW, helps eligible older adults and adults with disabilities receive long-term support in their homes and communities rather than in a nursing facility.
Depending on eligibility and an approved service plan, the waiver may provide access to personal assistance, support coordination, caregiver respite, home-delivered meals, emergency-response systems, assistive technology, and certain home modifications.
Eligibility is determined by the Louisiana Department of Health and includes medical and Medicaid-related requirements. Because every situation is different, families should speak with the appropriate state program representative to learn what services may be available.
